Does operating the A/C makes any difference?
What is the idling speed in Park/Neutral and when gears engaged?
Does transmission jerks when upshifting?
Did this occur after any repairs?

Idling speed should be at least 650 rpm when gears are engaged.

If idling in neutral is not as smooth as it shold be, engaging gears can result in stalling. Any difference when in R and D?

A fault in the transmission can cause this as well.

I would start wiht the basics, a tune-up and valve adjustment, igntion timing checks/adjustments.
