When the truck stands and press the gas pedal to the floor the first 35 or 40 mph feels a vibration, after 40 mph it gets smooth drive. I've aligned front end, replaced bad ball joints, replaced bad front ball bearings and replaced rack and pinion bushings. Thanks for your help.

Does the vibration shake the seats and floor board or the steering wheel, dash and pedals?

Remove the front drive shaft and drive it again and feels if it is gone, if so, have the driveshaft spin balanced. If the vibration is still there, get it about a two feet off the ground on a lift and start the engine place in gear and rev to 50 mph and again feel if it's gone, if not remove the rear drive shaft and if gone after that balance that shaft, if it's still there with either drive shaft it must be the engine or transmission.
If though it could also be the front suspension and with that a test drive is best by a mechanic.
