At low speed (under 50MPH) the truck drives fine. Once I get to about 65MPH or above the truck locks up for a split second and drives normal but will continue to lock up. The lock up is definitely coming from the front because I lurch forward towards the windshield. I had the transfer case replaced because I was unable to use the 4x4, it made all types of noise when I try to put in gear.

This can be the transfer case. I would remove the front drive shaft and see if the truck still it does it. If not, the transfer case will need to be rebuilt. Here is how to remove the front driveshaft. Check out the images (below). Please let us know what happens.

Just took it to transmission shop. It was confirmed that it is the rear. The left rear tire locks up once you hit about 60MPH.

Okay, we are getting closer, let's remove the rear tire to inspect the brakes. You could have a pad that is wedged between the rotor and mounting bracket. Check out the images (below). Let us know what happens and please upload pictures or videos of the problem.
