My 98 explorer has an engine miss that returns about every 4 to 5000 miles. It is the same plug the middle one on the driverside. it is not oil fouled but has carbon build up on it. Replace the plug and engine will run fine for a while. Also noticed a pulsating sound from the gas cap area the other day. Could a bad gas cap or carbon canister or O2 sensor cause this reoccurring problem.

Absolutely not. You need a lower fuel induction cleaning, preferably MotorVac, an upper air intake cleaning, and a new fule filter.
