1996 Dodge Caravan intermitent over heating

I believe I have a faulty fan control module, I have a new one but don't know where it is is located under the hood. Can you tell me where to look? Also I bought a new temperture sensor and need it's location too. The engine is a 3.0. It has a new thermostat, raditor and raditor cap.

sorry schematic is small but its located on the inner left front fender.
