1989 Chevy Truck stalling

I have a 1989 chevy c/k pick-up, it was sitting unused for about six years before I put it on the road. I was driving it the other day and then my engine light came on, and the check gauge light came on, and it started spitting and sputtering (like it was out of gas) and then it stalled. I could start it again but it kept on stalling. Just wondering what it may be from? I've changed the spark plugs, fuel filter, and the air filter. Please help

Try pulling the code/s from the computer and comeback with the code/s

I checked the codes and I think it came back as a code 12, 13, and 45. I'm not too sure how to read the flashes, but I think thats what they are.
