My car pull a big vaccume through the oil fill when car is running it also runs rough when the cap is off smooths out when closed

THat is normal if the PCV is working properly. It is designed to suck out pressure in the crankcase. If you open it, you create a leak. If the pressure was blowing out, that would not be good.
