Suzuki warning chime

My 2004 suzuki forenza headlight "left on" warning chime no longer works, is this a fuse or how do i fix it?

If the driver door switch is not working, the chime would not be activated. Ensure door ajar light is indicating when driver door is opened.

I checked Yes, the door ajar light is working correctly

CHIME MODULE DESCRIPTION
The chime module is located above the instrument panel fuse block and will sound in order to bring attention to one or more of the following conditions:

• The lamps are on, the door is ajar, and the ignition switch is not in ACC, ON, or START.
• The seat belt is unbuckled when the ignition switch is in ON or START.
• The door is open when the ignition switch is in ON or START.
• The key is left in the ignition switch when the ignition is in LOCK and the door is open.
Attached is a wiring schematic of the chime. Go through it and see what you can come up with.
