Transmission problem

This doesn't happen consistently. Sometimes when taking off from 1st gear, it's like the gear isn't in place or as if I still have the clutch down - the engine revs. 2nd gear is the same way. I was on cruise control and when going up an incline, the same thing happened; the engine revved like I had the clutch in rather than increasing speed. It's most notable on inclines and when the car is cold. Is it transmission or clutch and is it fixable?
