When driving everything seems fine until I come to a stop light and then a rattling sound comes from underneath the passenger side, it's where the exhaust goes through. I have checked to make sure it isn't loose and it isn't, but it only does this in drive or first and second after driving and comes to a stop still in that gear.

Good afternoon,

This sounds like the shield around the catalytic converter. Over time, the shield comes loose from the body of the converter and it rattles at idle. You could go under the truck and bang on the shield and I vet you hear the noise.I attached a diagram of the shield for you to view. This is the one that normally comes loose and makes the rattle.
