Either the heater core is plugged or there is a coolant flow problem, like a waterpump.

It is possible that thermostat is bad, but since one heater hose is hot and the other is warm, the core is plugged or the coolant flow is restricted.

Some waterpumps are fitted with a plastic waterpump impellers, these can seperate from shaft and reduce coolant flow.

My 2003 had that same problem a couple of years ago and it was a cracked radiator cap. So before you go spending a bunch of cash other things chack that!

Radiator cap is a possibility, it would be the least expensive item to start with, if problem persists, then look at waterpump or coolant flow restriction.
