1993 Plymouth Voyager Cooling Fan

The other day my car overheated and I noticed the fan wasn't running. I removed the fan and found that the motor has something inside (some part of the motor is moving around inside there) that prevents it moving freely. I replaced the fan with one from a salvage yard. They tested it at the yard and it worked. I installed but it won't come on even when I run the engine until it is quite hot--beyond the temp at which the fan would normally come on. Is there a fuse or relay some place that might have gone bad when the old fan became jammed? Thanks.

The relays for low and high speed operation are on the inner fender, the coolant temp sensor is probably the bad part tho...next to the thermostat.
