truck idles good then you start to drive put in gear and then the hesitation until you push in clutch and give it gas what could this be

Has the check engine light come on? Have you checked for vacuum leaks?

no the check engine light is not on

You may want to start checking for vacuum leaks. Also, do you know how to check for fuel pressure? If so do it. If you don't, let me know and I give you directions.

we changed the fuel filter checked all hoses and the fuel pump then we checked the manual transmission fluid and we did not have any in there could this be the problem

If there was no fluid in it, it may have been binding.
