Nissan Maxima Overheating Please Help!

Hi,
I'm at my wits end here my car is overheating it's not smoking however the temperature gauze says it's running hot and at times the car will jerk. I've replaced the radiator in January and I can't see any leaks. There is still coolant in my overflow bottle however I have to continuously pour coolant directly into my radiator! I will drive maybe 5-10 minutes then almost immediately my temperature gauze will shoot directly to the extremly hot signal. Any idea why?! What are signs that it may be a possible waterpump?

Remove radiator cap start car if the coolant is swirling around the water pump is okay-Try replacing the thermostat and then comeback when it continues.

Do you hear the cooling fan when it turns On?
