Make sure the PCV valve is working. If it is, you have what is called "Blow By". What that means is this. When engines start to wear out, the rings in the cylenders allow more and more compression to get past them which builds in the crank case of the engine then finds a way out through areas you mentioned. Make sure the PCV is clear and working before jumping to concolusions. Its job is to draw pressure from the crank case. Also make sure the hose going to the PCV is clear. I've seen them plug too.
