Even after adding coolant to my car it will only drive for approximately 5 minutes before reaching dangerous temperatures. Along with this the heat stopped working and the engine check light came on. This car has a rebuilt transmission and otherwise all original parts. Any ideas of possible problems?

heat stopped working

Sounds like the thermostat is closed preventing coolant flow-causing it to overheat. Better hope there's no constant white smoke on the back-if so its possible the headgasket is blown.
