I drive a 1998 Honda Accord EX Coupe. It is a 5 speed manual. 145K. 4 cylinder.

I had the timing belt, water pump, spark plugs and brakes repaired about 3 weeks ago. The car was running sort of loud before the repair. After the repair, the car had a vibration and squealing noise when starting/shutting off. I had the 3 of 4 motor mounts replaced yesterday which solved this problem. After the initial work and now after the motor mount replacement the car sounds worse than at did before....it runs very loud at 40mph+. The noise is a medium pitched hum and seems to be coming from the back of the car and Any ideas . . . I can't turn the radio up any louder to drown out the noise and it's driving me nuts!

There are a couple of joints on the exhaust system that could be leaking. The gasket could be failing OR you might have an exhaust system failure such as a rusted out pipe or muffler.

well not too many things in the back of the car to make noise. short of an exhaust leak you might have a wheel bearing going bad.
