2001 Honda Accord maintenance needs to improve engine perfo

Engine Performance problem
2001 Honda Accord 6 cyl Automatic 98000 miles

At 98000 miles, my Honda has had very little maintenance besides, brakes, oil changes and a new transmission 2 years ago. Do I need to have the transmission serviced? I know I need the timing belt changed. This is recommended at 75000 miles. What else do I need to have checked to improve mileage and performance, like tune up, etc.? It still has pretty good pick up, but not as good as it use to be. Also, I hear some major rattling when I go over rough parts in the road. I can hear this alot when I drive through the neighborhood or slow on a rough road. Sounds like it is right over the left front tire. Do you think it could be the struts? It sounds like something is fixing to fall off.
