2001 Ford Mustang V6 3.8L

I have a 2001 FORD MUSTANG 3.8L V6 COUPE 2D and has 79K miles. When I step on the Gas while driving I feel the shifting of the gears and the car gives a jerk. I have to really step hard on the gas to get the movement of the car. I changed the transmission fluid, Break Fluid and did the oil change recently.

Please let me know wat can be the problem

Is your car manual or automatic?
Just a thought, I have 2004 Mustang V6 (automatic)and when I have the gas pedal down to the floor and when the car starts to shift gears and the tach reads over 5000 rpm (redline is 5400), the car jerks heavily when shifting but doesn't when lower then 5000.
Are you letting the engine reach near redline when it is shifting? I would recommend letting off the gas a little at around 4500 rpm so it can shift properly (if its automatic).
