1991 Ford F-150 overheating

Coolent is full but boiling. Top hose is hot. I can't see or hear the fan clutch changing speeds. Should I change the fan clutch?

I would change the thermostat first if you havn't allready, its cheap and easy and make sure no pressure is on hoses/radiator and engine is cold. It may not be opening enough or just barely opening.

Never pour water in a hot radiator with car off.
You could run it without the thermostat just long enough to check how much flow your getting through the radiator. Take off radiator cap, start engine, top off fluid, you should be able to see in the radiator, and if it's getting good flow or if there is build up of deposits in coil paths restricting flow. If there is good flow, then your water pump is good and coils in radiator are clear.

Have you checked outer coils of radiator for bugs leaves, etc, good thing to spray them out if so. You do know not to put 100% antifreeze/coolant, 70% is the highest recommended, or it wont cool right.

Never open a hot radiator cap wait for it to cool.
Squeeze hose to see if pressure is on system before opening cap.

I would give this a shot first, then go for the clutch fan, it would'nt hurt to change it anyhow, their not too expensive.
