5.4L engine. Heater core is clear and open. I disconnected the hoses and ran water freely through the core. I reconnected the hoses and let the engine reach normal operating temperature. The inlet hose to the core was hot. However, the outlet hose was cool. The blower motor works fine. What could it possibly be? The heater core does not leak and the radiator does not lose water.

Even tho you believe the core is flowing freely, it's honeycomb design may be using only 10 percent of the cores capacity. As noted by the cold outlet. You need a high pressure flush, which works about 20 percent of the time or a new core. Works 100 percent of the time.
