My heater core has been replaced twice within the past year and a half my heat or air conditioning is working and my fan runs the whole time the car is on what can I do to get fix these problems?

My fan is always on and my heat nor the air conditioner is working I am having to refill my radiator with coolant at least twice a week

You have a leak in the coolant system
you need to pressure check the coolant system.

Check water pump
for leaks
