At cold start up it keeps dieing, unless I keep my foot on the gas. once moving, in feels like it is jerking back & forth and doesnt really want to go until i push down on the gas pedal harder, once I get up to about 60 mph is seems to run ok, whenever I push on the gas pedal a little harder it seems to want to go ok, but trying to stay at 30 mph(for example) it just starts jerking like its not running on all cylinders.

Dirty fuel injectors (cleaning the injectors often fixes this).
