2004 Dodge Caravan Overheat

I have a 2004 Dodge Grand Caravan, and it goes through antifreeze like crazy. I can fill it and within a few weeks it is completely out and is overheating. I don't notice it is going anywhere. And I am not sure what to do or where to check. Also my check engine light is always on. Is this a common problem with Dodge Caravans?

If you have a rear heater you may want to check the coolant line mounting bracket located under the van near the front passenger seat area. This is a steel bracket that connects the coolant lines traveling to and from the rear heater and is prone to rust through causing the coolant to leak out.
