Check engine light on my 1997 Town and Country stays on all of the time. DTC code (P0141) states that the problem is with the O2 senser heater circuit - Bank 1, sensor 2. If I disconnect and reconnect the battery the light will stay out for a few days then come back on. I have replaced both sensors and the problem doesn't go away.

Did you use a universal o2 sensor in the rear? May be a defective rear O2 sensor, connctor issue, or wiring issue outside chance of pcm problem but also possible.
Mitchell gives a troubleshooting tree on this.
