1996 Chrysler Town and Country shift problem diagnosis

This is a used trans. w/ approx. 105k to 120k mi. When it was first installed about 4 years & 50k mile ago, it woudn't always downshift to low when stopping. Upon accelerating it would make a clutch slipping kind of sound & then go. I've had no other problems & it has been serviced 3 times. including last month. Yesterday when it upshifted it seemed to go into neutral for about 3 seconds and then engaged. Since then it has not shifted up or down at all and still looses all drive for a couple of seconds occasionally when not fully warm. I've been told that I may need to replace the solinoid pack. What do you think?

is check engine light on ?
solenoid pack is a common problem on them tranny
usually no up shift and check engine on

Yes, there was a code for the output speed sensor. I put in another one & got rid of the code but still have the shift problem.. I have another solenoid pack I going to put it when it gets warm enough to do it outside.
