When I come to a stop the transmission will downshift with a clunk that jerks the car. The filter was changed about 1000 miles prior to this jerk. I put some additive to clean it, that stopped it for about two weeks. What is the cause of the clunk/jerk?

It could be a few things. First, has the check engine light ever come on? My first guess is a bad shift solenoid or the trans control module (TCM) could be the problem. However, they should set a check engine light.

Let me know.
