1995 Chevy Silverado Rough Idle And Stalling

My 1995 2500 Silverado has just developed an issue with with rough idling and stalling on start up. When the truck is started the engine drops immediately into a low rough idle, about 200-300 rpms, it will do this for about 20-30 seconds and then jump up to a normal idle. Sometimes it will drop down then go up and down in intervals a couple times before eventually staying at a normal idle. At first it was just the idle problem, but now it has started to die right after start up for the first two or three tries at starting it. The problem only occurs when the engine has been run up to normal temperature for awhile and then left to sit for about an hour, no problems occur at cold startups. I have already replaced the spark plugs, cap and rotor, and fuel filter.
