1995 S-10 Radiator Leak

I have a 1995 S-10 with 140,000 miles. It has the high output 4.3 and 4WD and is an automatic. Recently the hose that is located closest to the front of the truck and runs to the center of the engine has developed a leak right where it comes out of the radiator (I noticed a puddle on the ground and then opened the hood one day after running it and saw the fluid bubbling and coming out). I flushed it and added something to stop the leak, but today my truck started running extremely hot and when I got home (only 5 miles from work) I saw it leaking again in the same location and the other end of the hose was swelled up where it attaches closest to the engine. Also, my reservoir is completely crusted over with black dried up gunk. So there it is. Any help would be greatly appreciated.

Replace the hose, that'll fix the leak. Then flush out the radiator and engine. Add the orange dexcool antifreeze. But instead of adding water from the garden hose, mix it with distilled water. This will keep it from corroding up in the future.
