Engine Performance problem
1995 Chevy Monte Carlo Automatic

My car has more than 100,000 miles on it. It runs OK until it gets warmed up. Then, if I stop and start it again while it's still warm, it sort of jerks and stalls, sometimes actually dying, when I accelerate from a stop. It's done this off and on for several months. First, I got a tune-up with new plugs and plug wires and ignition coil. Then I got a new fuel pump. Each time, it ran OK for about a month, and then the problem came back. Now, it's doing it again, and I'm afraid to drive it very much. The low oil light comes on when it dies, but I just had the oil changed a week ago. I can't afford a new car right now, but I don't know what to do about this one. Any ideas what might be wrong?

Thanks. I forgot to add that a PCV hose (or connection?) Was also replaced because of a leak.
