2005 BMW M5 accelerator paddle

2005 BMW M5 V8 Four Wheel Drive Automatic

I was backing out of a parking lot when my accelerator paddle came off and the little rod that controls the speed is down and I can't get it out. How could this have happen and how do I fix it.

I'm talking about the gas paddle coming loose from the rod that's attached to it
