Twice within about a month, the fan blades have all broken off. Both times it occurred when making a rather quick start after stopping. We have been unable to identify any type of obstruction or foreign object causing the problem. What's the cause?

Get a good oem part, some subpar parts can handle the speed.

Is it an electric fan or belt driven?

Check the motor mounts and be sure they are not broken.
