2008 Chevrolet Uplander please help me

My uplander jerks when I take off from a stop and loseing power it seems to do it when the van warms up is there any way you could point in the right direction I have been tryin to get this fixed for a year now everyone keeps tellin me they can't find nothing a wrong with it

Scan for codes and check fuelpressure with a gauge. Clean throttle plate on both sides an diac hole with choke cleaner. If nothing shows up on scan have cam and crank sensor checked for resistance it may be to high and messing up when warm
