On starting 1.5dci. car runs very lumpy, car will not rev

After ticking over for 3 to 4 mins, car will rev but is still lumpy at tick over.. there is a lot of top end rattle.and fumes and smoke..

valve lifter are you talking camshaft.valve seals and what is a leakdown
