Spark Plug blow out

A spark plug blew on my 02 Celica. Plugs were changed at 120K and blew at 180K Car has been always dealer maintained. I am told two possibilities, either plug was over or under tightened. Should the dealer be held responsible?

With 60,000 mile's ago they did the plug's it's kind of hard to hold them responsible legally. Because if it was that bad of a job they did replacing them you would think it would have blown out a long time ago before 60,000 mile's went by. So are there any thread's left?They sell stainless insert's to repair the plug hole's. Let me know so I can help you out with this one.
