Fuel filler neck restrictions and fuel gauge behavior problems

Has anyone ever experienced a fuel tank filler neck restriction when fueling a 2001 Dodge Grand Caravan (no known accidents or reasons for a restriction with vehicle we have owned since 2003). Sporadically when you go to fill the fuel tank, the gas will back up into the neck very quickly causing the fuel nozzle to shut off. Using lowest pressure on fueling sometimes helps but sometimes nothing keeps the fuel from backing up to the top. It seems to happen more when the fuel gauge is reading more than a quarter full. Also, after we had owned the car for a couple of years, the fuel gauge would drop much more rapidly from half to empty as compared to full to half full. I don't know if this related or not. This is about the time we first noticed the restriction problem when we started seeing a change in the fuel indicator needle. The problem of restriction will occur several times then it will stop being a problem for an extended time. Any suggestions as the the problem?

Any applicable codes? I was thinking about the evap system or vent valve. The air in the tank has to go somewhere or it will burp back through the fill tube.

Have you tried a different gas station/different gas pump?
