Trouble codes Engine bank 1 & 2 lean and also sometimes cylinder misfire

I have changed out the fuel filter and checked for vacuum leaks not sure what to do next.

Could be one of the following below:

1. Vacuum leaks

2. EGR and PCV valve.

3. Low fuel pressure.

4. Dirty fuel injectors.

5. Mass Air Flow Sensor.

6. Oxygen Sensor.

I also forgot to tell you I removed all of the spark plgs one at a time and 4 of them looked good but the last two had black spots instead of that gray color, /what does that mean

Inspect the intake boot for tears or holes and consider that the factory recommends replacement of the oxygen sensors (pre-cat) at 100k

How do I get a software upgrade? I found the problem it was a tear is the oil dipstick vacuum hose.
