Surge then Stall

Engines starts when cold but once it's just under 1/2 way warm. It begins to surge then stall. If you hold your foot on the gas it will warm up then run fine? Vac pressure is 15inhg. No codes, no pending codes. I have changed everything. It only happens when the temp outside is below 0C and colder.

Check coolant
check for vacuum leak (what you have for reading is not an indication of no vacuum leak )
check IAC valve

Thankyou for the quick reply. For testing the IAC I have removed it and turned key on and seen that it cycles is this suffitient? For vacuum leak tessting I have sprayed ether around the intake runs and other vacuum areas, not lower intake though and had no increase in rpm. I replaced the lowerintake gasket and others above about 2 years ago.
