SOUNDS LIKE THE TENSIONER (WORN OUT) OR A PULLEY MAY NOT BE "TRACKING" IN-LINE (BEARINGS WORN OR BENT)----MAY EVEN HAVE ONE THAT'S NOT TURNING AT ALL (LOCKED UP)

YOU MIGHT REMOVE THE BELT AND SPIN STUFF BY HAND---PULL ON IT SIDEWAYS---ROCK IT AROUND TO SEE IF IT IS MESSED UP

SEEN WILD AND SEVERE NEGLECTED STUFF LIKE IN MY PIC BELOW
