Is the check engine light on? It could be the turbo beginning to fail. Get the turbo checked by a truck shop to verify.

Does it smoke and / or loose power? I have a 1999 K3500 doing a similar thing. Sometimes the turbo can be heard running and other times it does not sound like it's running at all. Low power at that time.

Any ideas?
