Dual fan relay?

I installed dual fan in my 1999 tahoe in this way

How can I start one fan by temp switch and AC?
Can I use 5 pin relay to do it? And how?

Do you have a working on/off temp switch in the engine to operate it from?

I mean a switch, not a temp sensor. The engine didn't come with one.
