Ok I need to know the name of the hose that has a 3 way plastic connector going from a raidiator hose to the bypass houseing, where the vaccum line going to the top of it has broken.

It is called the "Lower radiator hose" The "T" connects to the coolant reservoir bypass hose. All are available for view (not necessarily purchase) at this location.
