My transmission seems to be falling out of gear?

I have a 1998 Dodge Ram 4x4 with a 5.9L V8.. Recently, I found that when the engine was cold, it would not take of when first put in drive, it would rev, then lurch into gear, and when brakes were applied, it seemed to slip out of drive and then jump into gear again after gas was applied. I found I was low on fluid, and put some in and that seemed to correct the problem but then a few nights ago the weather got colder and the same thing happened.. Could it be the filter?

could be. change the fluid and the filter and see what happens. make sure you find the leak and repair that as well.
