1996 Ford F 250 Truck 5 8 351 Motor Get 45 Near Shakes Either Let Off Gas

My truck has a 5.8 351 motor and when I get to 45 or near, it shakes and I have to either let off the gas or throttle it. What is the problem and how can I fix it?
