What does code 12, and 36 mean for the 1995 toyota carina 7afe engine on the diagnostic trouble codes?

Car stalls when it is cold, does not seem like the choke kicks in to raise the revolution until it is warm, I have to depress the accelator pedal a few times before it moves, if not it cuts out when I try to move. I did the diagnostic checks and got codes 12 and 36 for the 1995 carina 1.8 7afe engine. Also while driving it will not rev all the way out to move at the correct speed, and then suddenly it accelarate quiclky and run well until you apply brakes and then it does it again. Yhanks for your advice.
