Clutch rod is falling off pivot

I just replaced the clutch master cylinder on my 95 ford f150 and it was running good until the clutch rod popped off inside. Is that known to happen? The clutch pivot arm that pushes the rod into the master cylinder from inside the firewall to the master under the hood. The rod attached to the pivot arm is where it pops off.

Check to make sure the clip is doing its job and is properly installed.
