Plymouth Voyager Rpm Problem

The engine had a squeak which started at low rpm and quieted at higher rpm's. The squeak got worse and eventually louder at high rpm. I checked the alt output read 12v. Last week changed alt no squeak but now its back just as it started. Tensioner less than a yr old. Any ideas?

new or used alternator??? belt tension correct??? what is the voltage at the battery???
