Lexus LS 400 Engine Problem
I have a 1994 lexus ls400 thats having an belt problem i changed the belt but that didnt work i changed the belt tensioner pulley and bracket but that didnt work either, so my question is what else could it be?

what is it doing? if it keeps coming off then it's an alignment problem and what engine
