I want an idea of what to look at and work on before I start tearing it apart. I've already changed the coolant and there was no difference.

First verify that the car is running hot and not just holding pressure. Make shore the seal on the cap is good. If it is overheating that fast the water pump may not be moving coolant. Things like the pump impelled may have rusted of or may be spinning on the shaft.it you remove the thermostat see if there is flow in the system when you reve the engine.
