Check oil cooler for cracks. Replace if found.
Make sure both mating surfaces are clean.
O-ring should aways be replaced after each disassembly. Never hurts to apply a little fresh oil to the new O-ring during assembly.
Align top cutout notch on oil cooler with protrusion on upper side of block. Tighten connector bolt to 36 ft-lb.
If still leaks, then I agree as mentioned adapter probably warped.
